I agree to assume full responsibility for any risks, injuries or damage known or unknown which my child may incur as a result of participating in the programs, classes or use of equipment.
Rise Girl™ has my permission to use my child’s photograph to share and promote the program for the goodness of all girls! I understand that no royalty, fee or other compensation shall become payable to me by reason of such use.
